1. ACCEPTANCE OF TERMS

The following terms and conditions (collectively the “TOS”) apply to your use of OHIPHCV.ca (“Site”, “we”, “our”, “us”). By using this Site, you accept and agree to be bound by the TOS herein. If the user (“you”) does not agree with the TOS, you should not access or use this Site.

We may modify the TOS from time to time without notice to you. After such change, you will be bound by the modified TOS. It is your responsibility to read and review the TOS periodically. If you do not agree to the changes made to the TOS then you must stop using the Site.

2. DESCRIPTION OF SERVICE

We provide users with an online health card validation Service (“Service”) to help improve accuracy and efficiency in your health care clinic. Included in this Service is the ability to access Ministry Of Health and Long-Term Care databases to retrieve patient information online in your web browser.

The Service does not provide medical, financial, legal, or other miscellaneous advice. All information on this Site is provided “AS-IS” and is solely for informational purposes. Validation checks eligibility and the health card number but does not guarantee payment of the claim since payment is dependent on additional factors. It also does not provide corrected or updated health card numbers or version codes. Therefore, the user assumes the entire risk with respect to the use of this information. The user must check the correctness of the data at their own end before submitting OHIP claims for reimbursement. Since data obtained is from the Ministry Of Health and Long Term Care databases, we cannot guarantee that the information is accurate, complete or up-to-date. We assume no responsibility for inaccurate content. What you decide to do with the information found on our Site is solely your responsibility and we are not liable for any of the actions you take as a result of using our Site.

The use of this Site is at your risk. We cannot guarantee that our Site will be available at all times and that they will be free of viruses or other harmful components. You are responsible for obtaining access to the Site and any of the fees associated with using the Site.

3. PRIVACY POLICY

4. NO COMMERCIAL USE

You agree not to reproduce, duplicate, copy, sell, trade, commercially exploit, or create derivative works from any content contained within this Site. This includes using the information found on our Site to further any commercial purpose through affiliate programs.

5. THIRD PARTY

This Site includes hyperlinks to other sites or resources. You acknowledge and agree that we are not responsible or liable, directly or indirectly, for any damage or loss caused in connection with these third party sites.

6. AVAILABILITY

The Ministry has scheduled downtime for maintenance every second Sunday of the month between 5:00 a.m. and 8:00 a.m. for their system. Our Service will be unavailable during this time and any other scheduled downtime by the Ministry.

7. AFFILIATE

We work in partnership with the Ministry of Health and Long-Term Care but do not have any financial interest in their departments. You, the user, accepts responsibility for maintaining an active GoSecure ID and GoSecure password in order to use this Site.
